On May 9, 2000, at around 4pm, Shiozawa fell down the stairway of his home. At the time, he thought he was okay and could shake it off, but at around 10pm, he suddenly fell unconscious and was rushed to the hospital. The next day, at 12:54am, he died of a cerebral contusion. He was 46. A Buddhist funeral has held at Housen Temple in Nakano, Tokyo.

In Detective Conan he was the original voice of Ninzaburo Shiratori, portraying him in the first four Movies as well as in his first two appearances in the anime (Metropolitan Police Detective Love Story and Metropolitan Police Detective Love Story 2). After his death, the role of Shiratori has since been performed by Kazuhiko Inoue, whom had co-starred with him in Touch and Blue Comet SPT Layzner. When Shiozawa was still alive, Inoue also played as Kyosuke Kazato in Captured in Her Eyes, a family friend of Shiratori's.
