"Silver Bullet" is the name given to a mysterious drug created by Elena Miyano and Atsushi Miyano. It is related to the APTX 4869 created by the Black Organization.

Background
19 years ago, the Miyanos were running the Miyano Clinic and not doing research but were considering accepting the Karasuma Group's offer to sponsor their research.[1] 17 years ago, revealed through cassette tapes Elena left behind for Shiho[2], they were working on a certain drug. While their lab mates were excited about it and considered it "a dream drug", Elena considered it "a truly terrifying drug" and the Miyanos were "calling it the Silver Bullet in hope" (perhaps in hope that it ends up as a drug for good rather than bad).[3][4] Elena and Atsushi had to leave behind Shiho and Akemi in order to complete the drug.[4]
After thinking about this message left by Elena, Ai Haibara regrets making APTX 4869 and thinks it is something that should have never been made.[4]
The research related to Silver Bullet was partially or wholly destroyed by the fire which also took the Miyanos' lives.[5] If some of it survived, it is possible that Shiho Miyano incorporated it into one of her modern projects. At the current time, it is still unstated how Silver Bullet, APTX 4869, and any other research projects of the Black Organization are related to one another.

Speculation

From the loving nature of the Miyanos, it is likely they had good intentions and were trying to create a benevolent drug. From what APTX 4869 became, it could be an anti-aging, de-aging or immortality drug. Elena's 18th tape suggests the drug development showed promise but they also realized the drug's potential to become something bad. APTX 4869 would therefore be a continuation of Silver Bullet in the bad direction. Shiho Miyano also hinted at two different possibilities for the drug when she said that she did not intend to create a poison,[7] and "what [the organization] wanted me to make, was another drug".
Haibara took over her parents' research after they died by reconstructing their work from surviving fragments of a lab fire which killed her parents.[8] One of these projects was the APTX 4869 project.
