Toshishiko is revealed to have behind this. He wanted to get back at Sayuri's father, who caused the death of his mother years ago; he had mistaken Toshishiko for a kid at a crime scene, not noticing his badly injured mother near by. In truth, Toshishiko originally only started dating Sayuri to get close to Matsumoto; however, he truly fell in love with her, something that left him hesitating.
It's later pointed out that both Toshishiko and Sayuri are the kids for each other's stories, and had unknowingly fallen back in love again after being seperated after so many years. This news leaves Toshishiko shocked.
As Sayuri didn't press charges, she and Toshishiko will get married after he spends some time in jail for his attempt on her life.

Manga to anime change
- In the manga, Conan rinses out Miss Matsumoto's mouth out with milk after he sees her collapsing, in an effort to neutralize the poison in her body. It is later stated that this bit of first aid may have saved Miss Matsumoto's life. These parts are cut from the anime entirely.
